MHMR provides service coordination to anyone in Tarrant County who is in the Home and Community-Based Services (HCS) Waiver, Texas Home Living (TxHmL), Pre Admission Screening and Resident Review (PASRR), and Community First Choice (CFC).
Our Service Coordination team provides you with a guide through the IDD programs and to other community resources.
Your service coordinator will provide assistance in accessing medical, social, educational, and other appropriate services and supports that will help an individual achieve a quality of life and community participation acceptable to the individual. Your coordinator will give you choices of which providers you have to choose from, including MHMR providers.
Working with the individual and his/her care givers, service coordinators develop a “Person Directed Plan” that reflects the desires and goals of the individual. Your service coordinator will help you locate those services that are identified in the plan.
Services may include support in the individual’s home or family home, day habilitation, employment assistance, supported employment services, services that assist the individual in accessing community activities, behavioral supports, or other services that may be available through other agencies. Respite or relief for the primary care giver is also available.

MHMR of Tarrant County also provides Case Management for Community Living Assistance and Support Services (CLASS) Waivers.
