Staff, we all do what it takes to make sure the clients are the priority. We have a trustworthy team and if we don’t the answer to a question or a problem, we work with the families and the Service Coordinators to
resolve the issues as quickly as possible. Finally, we have great relationships with several of the best Speech Therapist, Occupational Therapist, Physical Therapist and Dentist in Tarrant County.

Our agency participates at the monthly Arc Dances, our Day Habilitation clients and staff get in good exercise by walking the mall every Monday, we volunteer at one of the local food banks weekly, we bowl and have
fun at Alley Cats every Tuesday.
When we are not in the community we spend time at our Day Hab facility working on mock money management strategies. We learn simple easy snacks and meals to cook. We do arts and
crafts that hang around the day hab and on every Friday we have popcorn and movies.

What would you consider your greatest accomplishment in serving people with IDD?
|
Our greatest accomplishment in serving people with IDD is the growth of the client. Whether it’s a new mental breakthrough or a new physical breakthrough, we feel like we have done a ‘greater work’ when we hear
from the client or from the LAR/family/friends of the client, that they have improved in a certain area because of the time our staff took with them to listen or provide guidance or hand on hand direction. We don’t
thrive unless our clients thrive.

We have a combined 20+ years of experience in our Case Management and Direct Care positions. We have provided services to people with all different types of IDD. Our clients all have different medical backgrounds
and needs that we have been able to fulfill by working closely with their medical team and training staff on everyone. We currently have clients who have behavior plans and are seeing Behavior Specialist. We are
confident in our team’s ability to rise to any challenging or complex need.

Students and adults with Exceptional needs.

the complaint via telephone and/or face-to-face meeting, document resolution on
“Notification of Compliant” and, report resolution of compliant to HCS via

staff. Instead the client sees himself as part of the team. When the client sees
himself as part of the team, great things can be accomplished in the life of the
client.

Our staff has combined experience of over 15 years. Our organization was
started after the case management transition, therefore we were immune from some

wheelchair bound. They have g-tubes, tracheostomy and various other medical
conditions. Our staff is equipped to handle these cases.

Connie and Wendy are the owners and operators, combined we have over 50 years
experience working with the Intellectually Disabled. Connie grew up in a family
with an intellectually disabled uncle; she also worked as a state Surveyor,
investigating State Schools and group homes prior to opening up her first group
home in 1992. Wendy has extensive HCS experience, previously as the Program
Administrator of one of the largest HCS providers in Texas. Together with our
very experienced Case Managers, there is rarely a situation we have not
encountered or a problem we cannot solve.

Depends on the individuals, in our group homes, we have one home that has
Special Olympics 2 to 3 times a week, one home the girls are “honorary Dallas

family is unique and services are tailored around the individual’s unique needs.

Yes, we have a Day Habilitation program in South Grand Prairie off Lake Ridge
and Polo Rd. The name is OWL’s which stands for Opportunities to Work Learn and
Succeed. We contract with around 20 day habs in Tarrant County, Dallas County,
Ellis County, Johnson County and Collin County.

was, with the help of one of our families we went to Austin to speak in front of
the Senate to add the IDD population to the “Amber Alert”. Previously, an
individual with Intellectual Disabilities that went missing the Police would
only put out a “missing person” alert, which might mean that hours would go by
before the general public was alerted, which could be too late. With the
addition to the Amber Alert, agencies and the public are notified within 2 hours
if the individual cannot be located. The shorter the time, the better the
outcome. The Senate agreed and this became law in 2011.

And above all Honesty. We often tell families that we are not always going to

state and federal regulations that dictate these programs, it is important that
families work with a caring and knowledgeable provider that can navigate the
system, rather than a provider that will tell them what they want to hear and
then come back later and not deliver.

Any problems are taken up with the Case Manager, if the problem is not resolved,
they will bring it to Connie or Wendy and resolution we be accomplished. In

available. The 1st is payday so all staff and Foster Care families come by the
office to pick up their checks. It’s a great time to visit and be proactive
prior to any problems arising. Approximately every quarter, we will have a “Town
Hall” when or if something is coming up that the families need direction. Our
last town hall focused on “managed care” and how these changes would affect the
individuals we serve. We also have numerous social events, which includes a
family day at the lake each spring, a haunted house and Halloween party in the
fall and a large Christmas Dinner event and talent show.

Throughout our program, we use positive behavioral supports coupled with

successful community integration. Our day program called The Torch, is a social
and recreational programming model where we teach social and survival skills,
health and nutrition, hygiene and grooming skills directed at enhancing
individual lives. Daily, we access community venues such as libraries, parks,
malls, exercise classes, movie theatres, restaurants. Our enhanced and active
day program is an integral and important part of what we do at Champion
Services. Yes. We currently serve clients with complex mental health and
behavioral needs, persons with diabetes, persons with feeding tubes, persons
